Most doctors go into medicine with the broad aspiration to do good with their careers. This desire may coexist with various other motivations, such as intellectual engagement, pursuing scientific interests, or having a stable job and income (both of which can be instrumentally valuable for other goals), but it is nevertheless an important objective for many. We argue here that for doctors for whom impact is an important consideration, there are concrete steps that can be taken to have an impact that far outweighs what is achieved in a ‘typical’ clinical medicine career.
